Elderplan Plus Long-Term Care (HMO-POS D-SNP)

Get All Your Benefits Plus Long-Term Care in One Simple Plan 1

  • For Medicare and Medicaid (Duals), beneficiaries who qualify for Long-Term Care
  • Combines all your benefits, including medical, behavioral health, hospital prescription drugs and long-term care, into one simple plan.
  • Provides a personal care worker (PCW) to help you with daily activities plus attention from a Care Management team to help coordinate all your benefits.

Available in the Bronx, Manhattan, Queens, Brooklyn, as well as Westchester and Nassau Counties.

Elderplan Flex (HMO-POS)

Choose an additional OTC benefit and explore more ways to use your Flex Card.

  • For Medicare beneficiaries
  • For Medicare beneficiaries
  • Gives you the flexibility to select between an over-the-counter (OTC) OR transportation benefit to and from medical appointments.
  • Provides the convenience of medical, hospital and prescription drug coverage all in one plan.

Elderplan Extra Help (HMO-POS)

A health plan designed specifically for beneficiaries who are eligible who are eligible for Extra Help (LIS)2.

  • For Medicare beneficiaries
  • Get your medical, hospital, prescription drug coverage and more at little- or-no premium and with low copays
  • Provides additional benefits to help you stay healthy, like gym membership, wellness incentives, and access to Member-to-Member program

Available in the Bronx, Manhattan, Queens and Brooklyn.

  • For Medicare beneficiaries who live in one of Elderplan’s contracted assisted living communities, nursing homes, or congregate care settings.
  • Provides an additional level of care from our skilled Nurse Practitioner (NP) or Physician Assistant (PA) along with a dedicated Registered Nurse (RN)

Available in the Bronx, Manhattan, Brooklyn, Queens, and Staten Island, plus Nassau, Suffolk, Dutchess, Monroe, Orange, Putnam, Rockland, Westchester, Ontario, Orleans, Seneca, Yates, and Livingston counties.

Elderplan Advantage for Nursing Home Residents (HMO-POS I-SNP)

Receive Enhanced Care in a Nursing Home

  • For Medicare beneficiaries or dual Medicare and Medicaid beneficiaries who live in one of Elderplan’s contracted nursing homes.
  • Provides a skilled Nurse Practitioner (NP) or Physician Assistant (PA) who will support and guide you by working with your physicians to create a customized care plan.

Available in the Bronx, Manhattan, Brooklyn, Queens, and Staten Island, plus Nassau, Suffolk, Dutchess, Monroe, Orange, Putnam, Rockland, Westchester, Ontario, Orleans, Seneca, Yates, and Livingston counties.

  1. To be eligible for this plan, you must be 18 years or older, have Medicare and Medicaid, need community-based long-term care services for more than 120 days, and be assessed as nursing home eligible.[][]
