This plan is designed for Medicare and Medicaid beneficiaries who need valuable assistance with long-term care at home and want the convenience of their medical and prescription drug coverage in one plan. Elderplan for Medicaid Beneficiaries (HMO-POS D-SNP)

This plan is designed for Medicare Beneficiaries who also have Medicaid. We understand that coordinating your Medicare and Medicaid benefits can be challenging. That’s why we created a plan that makes your life a little easier by managing your medical, hospital, and prescription drug benefits at little-to-no cost. Available in the Bronx, New York, Queens, Kings, Nassau and Westchester counties.

Elderplan Flex (HMO-POS)

This plan is designed for Medicare beneficiaries looking for a zero premium plan that offers the flexibility to choose between benefits that matter most to you.

Available in the Bronx, Kings, New York, Queens, and Westchester counties.


Elderplan Extra Help (HMO-POS)

This plan is designed for Medicaid Beneficiaries eligible for Extra Help from Social Security. If you qualify for a low-income subsidy (LIS), you may be able to get assistance from the government with paying for your prescription drug coverage while you are enrolled in Elderplan Extra Help. And when you pay less for your Medicare coverage, you have more to spend on the things you enjoy.

Available in the Bronx, New York, Queens, Kings and Westchester counties.

Elderplan Select (HMO-POS I-SNP/IE-SNP)

A new health plan designed specifically for Medicare beneficiaries who live in one of Elderplan’s contracted assisted living communities, nursing homes, or congregate care settings.
It provides an additional level of care from our skilled Nurse Practitioner (NP) or Physician Assistant (PA) along with a dedicated Registered Nurse (RN) who will support and guide you by working with your physicians to create a customized care plan if needed, conducting preventive physical exams, managing chronic conditions, ordering lab tests, writing prescriptions, and answering your questions.

Available in the Bronx, Dutchess, Kings, Monroe, Nassau, New York, Orange, Putnam, Queens, Richmond, Rockland, Suffolk, Westchester, Ontario, Orleans, Seneca, Yates, and Livingston counties.

Elderplan Assist (HMO-POS IE-SNP)

This plan is designed for Medicare beneficiaries or dual Medicare and Medicaid beneficiaries who live in one of Elderplan’s contracted Assisted Living Communities. We know that receiving the care you need to continue living independently is important to you.

Available in the 5 boroughs of NYC, Nassau, Suffolk, Dutchess, Orange, Putman, Rockland, Weschester, Monroe, Livingston, Ontario, Orleans, Seneca and Yates counties.

Elderplan Advantage for Nursing Home Residents (HMO-POS I-SNP)

A health plan designed for Medicare beneficiaries or dual Medicare and Medicaid beneficiaries who live in one of Elderplan’s contracted nursing homes. HomeFirst Managed Long-Term Care (MLTC) Plan

A plan designed for individuals who receive Medicaid and require long-term care services and support at home. You can keep your own doctor as well as your Medicare and Part D coverage if you have it. There is no cost to obtain care from this plan. HomeFirst has been rated a 5-star MLTC plan for 3 years in a row*.

Available in the 5 boroughs of NYC, Dutchess, Nassau, Orange, Putnam, Rockland, Sullivan, Ulster and Westchester counties


Elderplan MAP is the only 5-Star plan in New York State. This is the highest rating a plan can achieve.*  The New York State Department of Health rates and rewards MAP plans based on their performance on quality measures related to patient safety, preventive screenings, quality of life, satisfaction, and overall health status.
