Elderplan for Medicaid Beneficiaries (HMO SNP): A Coordinated Care plan with a Medicare Advantage contract, but without a contract with the New York State Medicaid program that was created to help make sure Medicare beneficiaries who are eligible for Medicaid or another NY State medical assistance program¹⁸ get all the benefits they’re entitled to under both programs. Elderplan’s Medicare for Medicaid Beneficiaries (HMO SNP) provides benefits and/or value-added services that include:

• $36.40 monthly Part D plan premium¹¹
• $320 yearly Part D deductible
• Low co-pays for office visits to any in-network doctor, specialist or other healthcare provider^{13 18}
• $120 per quarter for over-the-counter, health-related items¹⁴
• Low co-pays for Part D prescription drug coverage¹²
• $3, 400 in-network maximum out-of-pocket limit
• Up to $100 for eyeglasses or contact lenses annually¹⁶
• Discounted acupuncture and massage services¹⁵
• No-cost gym membership at participating health clubs

All cost sharing is based on your level of Medicaid eligibility.
