Elderplan for Medicaid Beneficiaries (HMO SNP): A Coordinated Care plan with a Medicare Advantage contract, but without a contract with the New York State Medicaid program that was created to help make sure Medicare beneficiaries who are eligible for Medicaid or another NY State medical assistance program18 get all the benefits they’re entitled to under both programs. Elderplan’s Medicare for Medicaid Beneficiaries (HMO SNP) provides benefits and/or value-added services that include:
- $36.40 monthly Part D plan premium11
- $320 yearly Part D deductible
- Low co-pays for office visits to any in-network doctor, specialist or other healthcare provider13 18
- $120 per quarter for over-the-counter, health-related items14
- Low co-pays for Part D prescription drug coverage12
- $3, 400 in-network maximum out-of-pocket limit
- Up to $100 for eyeglasses or contact lenses annually16
- Discounted acupuncture and massage services15
- No-cost gym membership at participating health clubs
All cost sharing is based on your level of Medicaid eligibility.
