Elderplan Plus Long-Term Care (HMO SNP): A Coordinated Care plan with a Medicare Advantage contract and a contract with the New York State Medicaid program created for Medicare beneficiaries with full Medicaid who have chronic health care needs and are eligible to live in a nursing home, but would prefer and are able to live at home. With Elderplan Plus Long-Term Care (HMO SNP), you’ll get the following:
- $0 monthly Part D plan premium11
- $0 yearly deductible
- $0 visits to your network primary care physician13
- $0 visits to your network physician specialist13
- $3, 400 in-network maximum out-of-pocket limit
- $120 per quarter for over-the-counter, health-related items14
- $0 for inpatient in-network hospitalization13
- Transportation for medical services if medically necessary
- Personal care worker
- Home care
- Care management
All cost sharing is based on your level of Medicaid eligibility.
