With Elderplan Advantage for Nursing Home Residents (HMO SNP), you get:
- $36.40 monthly Part D plan premium11
- $320 yearly Part D deductible
- $3, 400 in-network maximum out-of-pocket limit
- Coordinate with physicians to create a personalized treatment plan
- Provide frequent follow-up visits
- Provide preventive care to avoid emergency room visits and hospital stays
- Help track unique medical conditions and make sure wishes for medical treatment and advance directives are followed
All cost sharing is based on your level of Medicaid eligibility.
