• $320 yearly Part D deductible
• $3, 400 in-network maximum out-of-pocket limit
• Coordinate with physicians to create a personalized treatment plan
• Provide frequent follow-up visits
• Provide preventive care to avoid emergency room visits and hospital stays
• Help track unique medical conditions and make sure wishes for medical treatment and advance directives are followed

All cost sharing is based on your level of Medicaid eligibility.
